Life in Quito is not-so-sterile as in US cities. March 2015.

Quito's streets are mostly clean, but there is chance to step on dog's or human's poop and to see guys peeing on a street in front of you.
When I lived in Centro Historico I saw (not often, may be a few times) mothers picking lice from their children's hair. Few times, in mom and pop restaurants, I saw cooks or waiters dropping food on the floor, picking it up and serving it to the customer later.
Some small eateries or stores might have pets hanging around while serving customers.
There are lots of carts in Quito, especially after 5 p.m., serving food and vendors, serving food from these cards, don't use gloves and they don't have an access to running water.

If you prefer sterile regulated environment Ecuador will disappoint you.
